EMAIL-ID / ईमेल आईडी : ENTER APPLICANT’S EMAIL ID. CERTIFICATE WILL BE SENT ON THIS EMAIL. आवेदक की ईमेल आईडी दर्ज करें। प्रमाण पत्र इस ईमेल पर भेजा जाएगा।

UAM NUMBER / यूएएम नंबर : ENTER THE APPLICANT’S 12-DIGIT UDYOG AADHAR MEMORANDUM (UAM) NUMBER FROM THE CERTIFICATE. प्रमाणपत्र से आवेदक का 12 अंकों का उद्योग आधार मेमोरेंडम (यूएएम) नंबर दर्ज करें।
